75f61fe3d9 AK: Make RefPtr, NonnullRefPtr, WeakPtr thread safe
This makes most operations thread safe, especially so that they
can safely be used in the Kernel. This includes obtaining a strong
reference from a weak reference, which now requires an explicit
call to WeakPtr::strong_ref(). Another major change is that
Weakable::make_weak_ref() may require the explicit target type.
Previously we used reinterpret_cast in WeakPtr, assuming that it
can be properly converted. But WeakPtr does not necessarily have
the knowledge to be able to do this. Instead, we now ask the class
itself to deliver a WeakPtr to the type that we want.

Also, WeakLink is no longer specific to a target type. The reason
for this is that we want to be able to safely convert e.g. WeakPtr<T>
to WeakPtr<U>, and before this we just reinterpret_cast the internal
WeakLink<T> to WeakLink<U>, which is a bold assumption that it would
actually produce the correct code. Instead, WeakLink now operates
on just a raw pointer and we only make those constructors/operators
available if we can verify that it can be safely cast.

In order to guarantee thread safety, we now use the least significant
bit in the pointer for locking purposes. This also means that only
properly aligned pointers can be used.

2cb32f8356 Kernel: Let InodeWatcher track child inode numbers instead of names
First of all, this fixes a dumb info leak where we'd write kernel heap
addresses (StringImpl*) into userspace memory when reading a watcher.

Instead of trying to pass names to userspace, we now simply pass the
child inode index. Nothing in userspace makes use of this yet anyway,
so it's not like we're breaking anything. We'll see how this evolves.

c8d9f1b9c9 Kernel: Make copy_to/from_user safe and remove unnecessary checks
Since the CPU already does almost all necessary validation steps
for us, we don't really need to attempt to do this. Doing it
ourselves doesn't really work very reliably, because we'd have to
account for other processors modifying virtual memory, and we'd
have to account for e.g. pages not being able to be allocated
due to insufficient resources.

So change the copy_to/from_user (and associated helper functions)
to use the new safe_memcpy, which will return whether it succeeded
or not. The only manual validation step needed (which the CPU
can't perform for us) is making sure the pointers provided by user
mode aren't pointing to kernel mappings.

To make it easier to read/write from/to either kernel or user mode
data add the UserOrKernelBuffer helper class, which will internally
either use copy_from/to_user or directly memcpy, or pass the data
through directly using a temporary buffer on the stack.

Last but not least we need to keep syscall params trivial as we
need to copy them from/to user mode using copy_from/to_user.

d89582880e Kernel: Switch singletons to use new Singleton class
MemoryManager cannot use the Singleton class because
MemoryManager::initialize is called before the global constructors
are run. That caused the Singleton to be re-initialized, causing
it to create another MemoryManager instance.

Fixes #3226

5e48eda218 Kernel: Avoid ninja-import of global variable
This would have caused an issue later when we enable -Wmissing-declarations, as
the compiler didn't see that Kernel::all_inodes() was being used elsewhere, too.
Also, this means that if the type changes later, there's not going to be weird
run-time issues, but rather a nice type error during compile time.

42b057b0c9 Kernel: Mark compilation-unit-only functions as static
This enables a nice warning in case a function becomes dead code. Also, in case
of signal_trampoline_dummy, marking it external (non-static) prevents it from
being 'optimized away', which would lead to surprising and weird linker errors.

I found these places by using -Wmissing-declarations.

The Kernel still shows these issues, which I think are false-positives,
but don't want to touch:

62ec6e5fe0 Kernel: Make Inode::read_entire() return a KBuffer (not ByteBuffer)
ByteBuffer is backed by kmalloc heap memory which is a scarce resource.
This fixes an OOM panic when traversing a large directory.

Kernel: Protect Inode list with SpinLock (#2748)
Fixes crashes when a context switch happens in the middle
of modifying it, or when another thread on another processor
modifies it at the same time.

6a74af8063 Kernel: Plumb KResult through FileDescription::read_entire_file() implementation.
Allow file system implementation to return meaningful error codes to
callers of the FileDescription::read_entire_file(). This allows both
Process::sys$readlink() and Process::sys$module_load() to return more
detailed errors to the user.

0fc60e41dd Kernel: Use klog() instead of kprintf()
Also, duplicate data in dbg() and klog() calls were removed.
In addition, leakage of virtual address to kernel log is prevented.
This is done by replacing kprintf() calls to dbg() calls with the
leaked data instead.
Also, other kprintf() calls were replaced with klog().

94ca55cefd Meta: Add license header to source files
As suggested by Joshua, this commit adds the 2-clause BSD license as a
comment block to the top of every source file.

For the first pass, I've just added myself for simplicity. I encourage
everyone to add themselves as copyright holders of any file they've
added or modified in some significant way. If I've added myself in
error somewhere, feel free to replace it with the appropriate copyright
holder instead.

Going forward, all new source files should include a license header.

ae64fd1b27 Kernel: Let symlinks resolve themselves
Symlink resolution is now a virtual method on an inode,
Inode::resolve_as_symlink(). The default implementation just reads the stored
inode contents, treats them as a path and calls through to VFS::resolve_path().

This will let us support other, magical files that appear to be plain old
symlinks but resolve to something else. This is particularly useful for ProcFS.

6bdb81ad87 Kernel: Split VMObject into two classes: Anonymous- and InodeVMObject
InodeVMObject is a VMObject with an underlying Inode in the filesystem.
AnonymousVMObject has no Inode.

I'm happy that InodeVMObject::inode() can now return Inode& instead of
VMObject::inode() return Inode*. :^)

c8e2bb5605 Kernel: Add a mechanism for listening for changes to an inode.
The syscall is quite simple:

    int watch_file(const char* path, int path_length);

It returns a file descriptor referring to a "InodeWatcher" object in the
kernel. It becomes readable whenever something changes about the inode.

Currently this is implemented by hooking the "metadata dirty bit" in
Inode which isn't perfect, but it's a start. :^)

08cd75ac4b Kernel: Rename FileDescriptor to FileDescription.
After reading a bunch of POSIX specs, I've learned that a file descriptor
is the number that refers to a file description, not the description itself.
So this patch renames FileDescriptor to FileDescription, and Process now has
FileDescription* file_description(int fd).
